Most check-in problems start on paper. A voucher with the wrong name spelling, no hotel confirmation number or a missing line about city tax costs the guest twenty minutes at the desk and costs you a phone call at 2am. Below are the fields that matter, a sample you can print, and an Excel template you can download.
Quick answer: A hotel voucher is the document a guest shows at check-in to prove the room is booked and paid. A usable hotel voucher format carries the hotel confirmation number, the lead guest name exactly as in the passport, dates and nights, room type, meal plan, what is prepaid and what is payable at the hotel, the cancellation terms and a 24-hour contact. Our free Excel hotel voucher template has all of these and prints to one A4 page.
A hotel voucher is proof of a prepaid stay. It tells the front desk three things: this guest has a reservation, someone has already paid for these parts of it, and here is who to call if anything doesn't match.
Three different parties can issue one, and a guest sometimes carries two:
That last point is where agency vouchers usually go wrong. The front desk doesn't care whose logo is at the top. It cares whether the name and the number on the paper match something in its system.
This is the field list we'd put on any hotel voucher, with the problem each one prevents. If you only fix two things on your current format, fix the confirmation number and the payable-at-hotel line.
| Field | Why it's there | What goes wrong without it |
|---|---|---|
| Hotel confirmation number (HCN) | The reference the hotel itself created for the booking. | The desk searches by name, finds nothing under the guest's surname and assumes there's no booking. |
| Supplier or portal reference | Lets the hotel or your supplier trace the booking from their side. | Your 2am call to the supplier takes ten minutes longer. |
| Lead guest name | Surname and given names exactly as in the passport. | "Bob Smith" on the voucher, "Robert James Smith" in the system. Usually solvable, always slow. |
| Other guests and child ages | Occupancy is priced. A child of 13 is often an adult to the hotel. | An extra-person charge at check-in that nobody quoted. |
| Check-in, check-out, nights | Written as 14-Mar-2027, never 03/14 or 14/03. | A guest from the US reads 04/05 as April, the hotel in Penang reads it as May. |
| Room type and number of rooms | The category that was actually booked. | An upgrade argument, or a guest who thinks "family" meant two rooms. |
| Meal plan | RO, BB, HB, FB or AI, plus how many people it covers. | Breakfast charged for the second guest because the voucher said "breakfast" without "for 2". |
| Bed type | Marked as a request, not a promise. | A honeymoon couple shown to two single beds, holding a voucher that said "king bed". |
| Prepaid by agency | Exactly what has been paid: room, meal plan, taxes or not. | The hotel charges the guest for the room as well. |
| Payable by guest at hotel | City tax, resort fee, tourism fee, deposit, extras. | An angry guest, and a refund request that lands on you rather than the hotel. |
| Cancellation terms | Refundable or not, the free-cancellation date and the charges after it. | A client cancels late and swears nobody told them. |
| 24-hour contact | A phone number that is answered at night. | The guest pays the hotel again because nobody picked up. |
| Issue date and voucher number | Shows which version the guest is holding. | Two versions in circulation after a date change. |
The payable-at-hotel line deserves its own sentence. Dubai, for instance, adds a Tourism Dirham fee of AED 7 to 20 per room per night depending on hotel classification (UAE government portal). It's small, it's normal, and it causes more check-in complaints than almost anything else when nobody mentioned it.

Download the hotel voucher templatePeople search for a hotel voucher format in Word mainly because Word is what's open on the desk. That works, but you'll be calculating nights by hand and fighting table borders. Our suggestion: keep the master in Excel, then either save to PDF for the client or paste the filled range into Word if your agency's letterhead lives there. A PDF is the right thing to send. It can't be edited by accident, and it opens the same on every phone.
You'll recognise most of these from support calls.
One rule worth keeping: whatever voucher the client holds, its guest name and hotel confirmation number must be copied from the supplier booking, character for character. Everything else on the page is presentation.
When you book a hotel on the portal you can download a PDF voucher from the booking page. To be clear about what it is: it's issued under Travel DMC Group, the name of our group, with our 24/7 emergency line (+65 8034 8190). It is not white-label, so it won't carry your agency logo.
What it shows:
If you'd rather the client sees your brand, fill our template with the same guest name and confirmation number and send that, keeping the portal voucher as your supplier document. It's a common arrangement and hotels handle it fine as long as the two match. For the cancellation side, the hotel cancellation policies guide explains how to read the charge steps, and the meal plans guide decodes RO, BB, HB, FB and AI.
A limitation worth knowing: special requests you enter at booking don't print on our voucher, and they're requests rather than guarantees anyway. Put them on your own client voucher so expectations are set in writing.

At minimum: the hotel confirmation number, lead guest name exactly as in the passport, hotel name and address, check-in and check-out dates with the number of nights, room type, meal plan, what has been prepaid and what the guest pays at the hotel, the cancellation terms and a 24-hour contact number for the issuing agency.
Not quite. A confirmation means the hotel has accepted the reservation, identified by its confirmation number. A voucher is the document the issuing agency or wholesaler gives the guest to show the stay is booked and paid. A good voucher carries the hotel confirmation number, which ties the two together.
Most city hotels are happy with a PDF on a phone, and many never ask for the voucher at all because they find the booking by name. Some resorts and smaller properties still prefer a printout, so we suggest guests carry both.
Yes. You can rebuild the layout as a Word table, or copy the filled range from our Excel template and paste it into Word. Excel is easier because the nights calculate themselves and the sheet is already set to print on one A4 page, which you can save as PDF.
It is the reference the hotel itself created for the booking, sometimes called the HCN. It is different from the agency or supplier booking reference. If a voucher shows only a supplier reference, the front desk may not be able to find the booking quickly.
No. Vouchers from the portal are issued under Travel DMC Group with our 24/7 emergency line. Many agents give their client a voucher in their own branding, built from a template like ours, and keep the portal voucher as the supplier document.
